Transgender ID Change in Wisconsin
Wisconsin State ID Change
WI is ambiguous on requirements to change the marker. An affidavit or statement from a medical doctor or director of a facility specializing in gender change providing the customer is enrolled in a program may suffice. However, a court order or physicians report may be required.
If you have legally changed your name, you will need to change your name with the Social Security Administration Office before coming to a DMV Service Center. When you come to a Service Center to change your name on your Driver License or Identification Card, you will be required to show proof of your name change. The following are acceptable documents to show proof of your name change:
- A certified record of marriage/divorce
- Certified court order
The current fee for a duplicate card is $14 for a driver’s license and $16 for an identification card.
Wisconsin Birth Certificate Change
Wisconsin will change name and sex on the birth certificate. You will need a certified copy of your Court Order for Name Change as well as a certified copy of a Court Order for change of gender (two separate court orders). You will need a signed, dated, notarized letter from your surgeon confirming the date of the procedure. The surgeon should include your name, date of birth, date of surgery, type of surgery (male-to-female), and where it was performed. Finally, you will need your original birth certificate or a certified copy.
