Transgender ID Change in Connecticut
Connecticut State ID Change
Changing the gender marker in Connecticut requires a letter from a doctor stating that genital realignment surgery has been performed, or a letter from a health care provider attesting that the applicant is in active treatment and is living full time in the target gender according to the Standards of Care. In order to change your name on your driver’s license or identification card, requires your current driver’s license along with a certified copy of your name change court order. A new license will be issued at no cost, indicating the new name.
Connecticut Birth Certificate Change
To change your name you need to get a Probate Court order for the name change and take it to to the town clerk of the town where you were born. They will AMEND your Birth Certificate.
To change your gender you must contact the Vital Records office in the State Department of Health in Hartford. You can contact Maria Colon at:

They will send you two affidavits you must have completed and NOTARIZED. One affidavit is for a therapist to confirm your female identity and the other is for the SRS surgeon to confirm you had SRS. If you had SRS outside the United States, you’ll have to get an affidavit from a US Doctor confirming that SRS was done.
