Transgender ID Change in Ohio

Transgender ID change: How to change your driver's license, id cards and birth certificate in the U.S.Ohio, as of September 2009, has changed its policy on gender change and no longer requires a statement of surgical changes.

A new form, “Declaration of Gender Change“, requires a signed statement from a physician or licensed therapist that the applicant is undergoing “gender change” in accordance with the Standards of Care. The exact text used is “It has been determined this individual is sufficiently ready for, or has completed gender role transition, and it is intended for this role change to be permanent. This transition may or may not lead to further surgical intervention.”

The forum is available by calling the main office of the Ohio BMV. (614-752-7500) This process will need to be repeated each time the license is renewed until the transition is complete. Once the form is completed, it is mailed, or faxed to the BMV.

Notification of the change

Notification of the change will be mailed to you, which is then taken to your local BMV office to get a new license issued. The old license must be surrendered. The cost is the same as a duplicate license.

After a legal name change, you will need to take your drivers license, certified name change court order, and current Social Security card into a Deputy Registrar’s office and purchase a new license with the name change. Changing your Social Security record is recommended before correcting your driver’s license or ID card. The fee for a duplicate license is $19.50.

Ohio Birth Certificate Change

Ohio will NOT change the sex on the birth certificate. There was some question whether Ohio will even change the name, but the latest information reveals that name change can be done through the Division of Vital Records with a court order. This, however, does not amend the original birth certificate. An “abstract copy” will be created with a court order, and if you request by name this “abstract copy” you will receive an attached statement with your new name.
